“Smile design” is the marketing term of recent years in aesthetic dentistry. But beyond the attractive phrase, there’s a real technical process that differentiates quality aesthetic intervention from random work. Here’s what it really means.

What smile design is

Smile design is the process of planning the final look of your smile before starting any invasive procedure. It combines:

  • Facial analysis — your face proportions dictate tooth proportions
  • Dental analysis — current shape, length, colour of teeth
  • Function — occlusion must remain correct, not just aesthetic
  • Your expectations — what you want to change, reference
  • Digital simulation or physical mock-up — you see the result before accepting it

It’s not a single treatment. It’s the architecture of a complete plan that may include whitening, veneers, crowns, orthodontics, and gum aesthetics.

Why the process matters

Without proper smile design, risks:

  • Veneers that don’t fit the face — teeth too large, too white, too symmetrical, unnatural “Americanised” look

The 7 principles of smile design

Modern aesthetic dentistry is based on well-defined principles — not on “what the doctor thinks is beautiful”:

1. Midline

The line that separates central incisors must coincide with the face axis (line through the middle of nose, lips). Large asymmetry = unnatural look.

2. Dental proportions (Golden Proportion)

The width of visible teeth gradually decreases from centre to side, in specific proportions. Ratio 1.6:1 or 1.6 between central and lateral incisor. Modified by modern dentists to 0.62-0.65 for more natural appearance.

3. Gum line

The gum contour must follow a harmonious curve. Asymmetric recessions or hyperplasias spoil the appearance — sometimes corrected with aesthetic gingivectomy or gum lifting.

4. Central incisor length

Upper jaw central incisors should be the longest frontal teeth. Length/width ratio of an incisor: 75-80%. Less = “short” teeth; more = “horses” (unnatural long look).

5. Smile line

The upper contour of lower teeth (when smiling) should follow the lower lip line. Parallel lines = harmonious smile.

6. Tooth colour

Must be natural for your age and face tone. Too bright white looks artificial (“Hollywood smile” effect). We use the VITA shade guide — A1, A2, B1 are natural, bleach shades (BL1-BL4) are very white.

7. Tooth shape and individual characteristics

Teeth aren’t perfectly symmetrical. Small irregularities, mamelons on edges, fine enamel lines — all these details make the difference between “obvious veneers” and “naturally beautiful teeth”.

Smile design process — step by step

1. Initial aesthetic consultation (45-60 min)

  • Discuss expectations — bring photos of smiles you like (actors, friends, or yourself when younger)
  • Complete clinical examination
  • Professional dental photos: front with smile, side, retractors raised for details
  • Facial photos (with and without smile)
  • Honest discussion about what’s possible and what’s not

2. Digital documentation (1-2 sessions)

  • 3D intraoral scan — exact digital model
  • VITA shade guide — document current shade
  • Panoramic / CBCT 3D X-ray (case-dependent)
  • Occlusion recording (which teeth touch when biting)

3. Digital design (DSD — Digital Smile Design)

Dental engineer + specialist work on your digital images for:

  • Sketch new smile over your photo
  • Modify tooth shapes and lengths
  • Symmetry and proportion test
  • Final result simulation (photo + 3D)

At the second consultation you receive the visual simulation of your new smile.

4. Physical mock-up (“smile try-out”)

For complex cases, we do a mock-up directly on your teeth:

  • Apply transparent composite over current teeth, shaped to design
  • See in mirror the real result (with eating, talking, smiling)
  • 1-2 days to get used and decide
  • Accepted or adjusted until you’re 100% satisfied

This is the point of no return. Your final approval starts the invasive phases.

5. Preparation phase

  • Professional hygiene
  • Whitening (if whitening brings natural teeth close to final veneer colour — fewer veneers needed)
  • Orthodontics (if mild alignment changes the plan)
  • Conservative treatments for existing cavities
  • Possibly aesthetic gingivectomy

6. The actual implementation

  • Minimal tooth grinding for veneers (under local anaesthesia)
  • Exact digital impression
  • Provisional veneers identical to approved mock-up — worn 2-3 weeks
  • Bonding of final ceramic veneers (1-2 sessions)
  • Occlusal finishing

7. Post-treatment maintenance

  • Night protection tray (mandatory if you grind)
  • Check-ups every 6 months
  • Hygiene with soft brush, avoiding aggressive bicarbonate
  • Attention to trauma (ice, opening bottles)
  • Aesthetic maintenance (polishing 1-2 times/year)

Who’s an ideal candidate for smile design

Ideal candidate:

  • Adult over 18 (permanent teeth fully erupted)
  • Healthy gum, no active periodontitis
  • Realistic expectations — wanting improvement, not Hollywood perfection
  • Budget for long-term investment (15,000-50,000 RON)
  • Discipline for post-treatment maintenance

Can I see the simulation before deciding? Yes. That’s the entire purpose of smile design — you see the result (digital + physical mock-up) before any irreversible procedure.

How long does the result last? Quality ceramic veneers: 15-20+ years with proper care. Whitening needs refresh at 1-2 years. Ceramic crowns: 10-15 years.

Can I get smile design if I grind my teeth (bruxism)? Yes, but mandatorily wear a protection tray at night. Without it, veneers can crack within 1-2 years. We also discuss treatment of bruxism cause (stress, altered occlusion).

Will I have sensitivity after veneers? Possibly temporarily (1-3 weeks). For cases with chronic sensitivity we use specific adhesives and desensitising gel.
